Best Hospitals in India: Delhi vs Mumbai vs Chennai vs Bangalore — City Comparison for International Patients
Choosing the right city in India for your medical treatment is almost as important as choosing the right hospital. India's four major medical cities — Delhi, Mumbai, Chennai, and Bangalore — each have distinct speciality strengths, cost profiles, climate conditions, and levels of infrastructure for international patients. Getting this choice right can mean the difference between a smooth, comfortable experience and unnecessary complications.
This guide is designed to help African patients and their families understand the practical differences between India's top medical cities and make an informed decision before travelling.

Delhi and the National Capital Region
Delhi's National Capital Region (NCR), which includes Gurgaon, Noida, and Faridabad, is India's undisputed capital of medical excellence for international patients. The sheer concentration of top-tier hospital brands — Apollo Delhi, Medanta, Fortis Gurgaon, Max Saket, and BLK-MAX — creates genuine competition that benefits patients through pricing and quality.
Delhi's Speciality Strengths
Cardiac surgery is where Delhi excels most clearly. Medanta's Heart Institute, Escorts Heart Institute (now Fortis), and Apollo Delhi perform among the highest volumes of cardiac procedures in Asia. TAVI (transcatheter aortic valve implantation), complex valve repairs, and heart transplantation are all available with outcomes matching European benchmarks.
Neurosurgery and neurology at AIIMS Delhi and Medanta are globally competitive. Deep brain stimulation for Parkinson's disease, awake craniotomy for brain tumours, and epilepsy surgery are available from surgeons with international training.
Organ transplantation — liver, kidney, heart, and lung — is best resourced in Delhi, with the largest transplant teams and shortest waiting times for cadaveric organs.
Haematology and bone marrow transplant: AIIMS and Medanta lead India for allogeneic bone marrow transplantation. Learn more about bone marrow transplant in India.
Delhi Considerations
Delhi's summer heat (May to June: 42°C+) requires careful planning for patients who need to recover outdoors or have cardiovascular conditions. Air quality in winter (November to February) can be poor. The city is large and traffic is significant, so choose accommodation within 2km of your hospital.
Mumbai
Mumbai is India's financial capital and second-largest medical destination. The city is home to Tata Memorial Hospital (Asia's largest dedicated cancer centre), Kokilaben Dhirubhai Ambani Hospital, and Breach Candy Hospital, among others.
Mumbai's Speciality Strengths
Oncology is Mumbai's defining strength, led by Tata Memorial Centre which handles over 65,000 new cancer cases annually. Complex head and neck surgery, hepatobiliary oncology, and haematological malignancies are treated at volumes that create unmatched surgical experience.
Cardiac surgery at Kokilaben and Asian Heart Institute is excellent, with specialisation in minimally invasive and robotic cardiac procedures.
Orthopaedic surgery, particularly complex revision joint replacement and sports medicine, is available at Breach Candy and Nanavati Max hospitals.
Mumbai Considerations
Mumbai is India's most expensive city for hospital care. Accommodation costs are higher than Delhi or Chennai. The city's coastal humidity makes post-surgical recovery management important — air conditioning is essential. International flight connectivity is good with Ethiopian Airlines, Air India, and Emirates.
Chennai
Chennai has built a well-deserved reputation as India's most affordable and internationally patient-friendly medical city. The city has a long history of international medical tourism, particularly from Sri Lanka, Southeast Asia, and increasingly Africa.
Chennai's Speciality Strengths
Proton therapy and oncology — Apollo Proton Cancer Centre is the only proton therapy centre in South Asia, making Chennai the only destination in the region for paediatric brain tumours and prostate cancers requiring proton treatment.
Cardiac surgery at Apollo Chennai and Sri Ramachandra Medical Centre offers comparable outcomes to Delhi at 10 to 15% lower cost.
Ophthalmology and corneal surgery at Sankara Nethralaya, one of the world's leading eye hospitals, is a unique Chennai offering.
Orthopaedics and joint replacement at MIOT Hospitals, long regarded as India's leading orthopaedic institution, brings specialist experience in complex revision surgery.
Chennai Considerations
Chennai's temperatures are moderated by sea breezes compared to Delhi's intense summer heat. The city is more compact than Delhi, making it easier to navigate. Tamil Nadu's healthcare infrastructure for international patients is mature, with well-established airport arrival assistance and hospital coordination services.
Bangalore
Bangalore, India's technology capital, has a growing healthcare sector that combines world-class robotic surgery capabilities with a more pleasant climate than other medical cities.
Bangalore's Speciality Strengths
Fertility and IVF — Bangalore has a concentration of high-quality fertility centres including Milann and Manipal Fertility, appealing to the city's educated professional demographic and now attracting international patients.
Robotic surgery across specialties is particularly well-developed in Bangalore, where hospitals compete for the city's tech-savvy patient base.
Organ transplantation at Manipal Hospitals Bangalore is growing, particularly kidney and liver transplant.
Bangalore Considerations
International flight connections from Africa are less direct than Delhi or Chennai, typically requiring a connection in Mumbai or Delhi. The city's climate — mild year-round at 25 to 32°C — is the most comfortable in India for recovering patients.
Making Your Decision
The right city depends primarily on your procedure. Use this framework:
- Cardiac surgery or transplant: Start with Delhi, then Chennai
- Cancer treatment: Chennai for proton therapy; Delhi for haematology; Mumbai for head/neck surgery
- Orthopaedics: Chennai (MIOT) or Delhi equally strong
- IVF and fertility: Bangalore or Delhi
- Neurosurgery: Delhi (AIIMS, Medanta) as first choice
Once you identify the right city, Arodya's coordinators will shortlist two or three hospitals within that city for your specific procedure, obtain quotes, and help you choose based on surgeon experience and cost.
Complete the Arodya intake form to get a personalised city and hospital recommendation based on your diagnosis, budget, and travel preferences. We have coordinated treatment for African patients across all four cities and can advise from real experience.
Travel and Logistics Notes
All four cities have international airports. Direct flights from East Africa to Delhi (via Ethiopian Airlines, Kenya Airways) are most frequent. West African patients typically route via Addis Ababa or Casablanca to any of the four cities, with Delhi and Mumbai having more frequent onward connections.
Visa documentation for any of the four cities is the same — an Indian medical visa (MED) requires a hospital appointment letter, which Arodya secures as part of the facilitation process.
