Shoulder Arthroscopy in India for Sports Injuries: Rotator Cuff, Labrum Repair — Costs and Recovery

Shoulder Arthroscopy in India for Sports Injuries: Rotator Cuff, Labrum Repair — Costs and Recovery
The shoulder is the most mobile joint in the human body — and consequently one of the most injury-prone. Athletes in football, rugby, volleyball, swimming, cricket, and virtually every other sport are at risk. But shoulder injuries are not limited to professional athletes. Heavy manual labourers, construction workers, farmers, and anyone who uses their arms overhead repeatedly are susceptible.
When conservative treatment — physiotherapy, injections, rest — fails to resolve a shoulder injury, arthroscopic surgery offers the most effective path to restoring function and eliminating pain. India's sports medicine and orthopaedic centres provide this surgery at a fraction of Western costs with outcomes that match the best international standards.
Understanding Shoulder Injuries: Common Conditions
Rotator Cuff Tears
The rotator cuff is a group of four muscles (supraspinatus, infraspinatus, teres minor, subscapularis) and their tendons that stabilise the shoulder joint and allow arm elevation and rotation. Tears occur acutely (traumatic fall) or progressively through degeneration (overuse).
Types:
- Partial thickness tear: Tendon fibres damaged but not completely severed
- Full thickness tear: Complete breach through the tendon
- Massive tear: Large or multi-tendon tears, requiring complex reconstruction
Symptoms: Pain with arm elevation (especially overhead), weakness, night pain, and a "clicking" sensation.
Labrum Tears (SLAP tears and Bankart lesions)
The labrum is a ring of fibrocartilage that deepens the shoulder socket and provides stability.
- SLAP (Superior Labrum Anterior to Posterior) tear: Tear of the top portion of the labrum where the biceps tendon attaches. Common in overhead athletes (volleyball, cricket bowling, swimming). Causes deep shoulder pain and clicking.
- Bankart lesion: Tear of the front-bottom labrum, occurring after shoulder dislocation. Leads to recurrent dislocation if not repaired.
- Posterior labrum tear: Less common, associated with contact sport injuries.
Shoulder Impingement
Pinching of the rotator cuff tendons beneath the acromion (bony arch) during arm elevation. Causes pain with overhead activities. Often accompanied by subacromial bursitis.
Shoulder Instability (Recurrent Dislocation)
After multiple dislocations, the shoulder capsule and labrum become lax. Arthroscopic capsular plication (tightening) or Latarjet procedure (bone block) restores stability.
Biceps Tendon Pathology
The long head of biceps tendon can become inflamed, partially torn, or unstable in its groove. Biceps tenodesis (reattachment) or tenotomy (release) is performed arthroscopically.
Acromioclavicular (AC) Joint Injuries
Separation of the AC joint from direct impact (shoulder tackle, fall). Grades III-V typically require surgical reconstruction.
Diagnostic Imaging for Shoulder Surgery Planning
Before any shoulder surgery in India, your surgeon needs:
MRI shoulder (with or without arthrogram):
- Standard MRI: Adequate for most rotator cuff tears
- MRI arthrogram (contrast injected into joint): Superior for labrum tears and partial rotator cuff tears. If you are coming for labrum repair, request an MRI arthrogram from your local radiologist.
X-ray shoulder series:
- AP view, supraspinatus outlet view
- Assesses bone, calcification, AC joint separation grade
Indian surgeons can review your MRI files (DICOM format) remotely through Arodya, provide a surgical recommendation, and confirm whether the planned procedure matches your injury pattern before you travel.
Arthroscopic Procedures: What Happens in the Operating Theatre
Shoulder arthroscopy is performed under general or regional anaesthesia with the patient in a "beach chair" or lateral decubitus position. The surgeon makes 2-4 small incisions (portals) of 5-7mm.
Rotator cuff repair:
The torn tendon is reattached to the humeral head using suture anchors — small titanium or biodegradable devices that hold sutures within the bone. The tendon is pulled down and sutured to the anchor threads. Small tears require 1-2 anchors; massive tears may require 4-6 anchors plus additional techniques (augmentation with graft if necessary).
SLAP repair:
Suture anchors placed at the top of the glenoid reattach the labrum and biceps anchor. The number of anchors depends on the tear extent.
Bankart repair:
Anchors placed at the front-bottom glenoid rim reattach the torn labrum, recreating the "bumper" that prevents anterior dislocation.
Subacromial decompression:
A small amount of the acromion is shaved away (acromioplasty) to create more space for the rotator cuff tendons, reducing impingement.
Biceps tenodesis:
The long head biceps tendon is detached from its anchor at the top of the glenoid and reattached to the humerus at a lower position using an anchor or tenodesis screw.

Recovery and Rehabilitation: What to Expect
Shoulder surgery recovery requires a structured physiotherapy programme. Phase-based rehabilitation:
Phase 1 (0-6 weeks): Protection
- Arm in sling
- Passive range of motion exercises only
- Pain and swelling management
- No active muscle use of operated shoulder
Phase 2 (6-12 weeks): Active recovery
- Active assisted and then active range of motion
- Gradual strengthening begins
- Sling discontinued
Phase 3 (3-6 months): Strengthening
- Progressive resistance exercises
- Sport-specific movement patterns
- Return to light sports activities
Phase 4 (6-9 months): Return to sport
- Full sports return for contact and overhead sports
- Independent gym programme
India's hospital physiotherapy departments provide a full post-operative rehabilitation programme during your India stay, plus a written protocol for you to continue at home. For patients returning to Africa, we recommend identifying a physiotherapist in your home city before you travel so they can receive the protocol and continue your rehab seamlessly.
